Appointments

St. Lukes uses an advance appointment system which helps minimize the waiting period. Please make your appointments in advance at the Appointment Services Desk.

Information required for making appointments

For First visit patients,

  1. Mention your name, birth date, address, and telephone number.

For Revisit patients,

  1. Have your hospital ID number ready.
  2. Make a note of your appointment number that we will provide.

Rehabilitation 03-5550-7062 Weekdays 9:00 A.M. – 4:00 P.M.

  • Patients without appointments may not be able to see the doctor in some departments on the same day.
  • Walk-In-Clinic and Emergency Medical Care Center (the ER) provides walk-in services.
  • We will make every effort to accommodate your schedule, but since limited numbers of appointments are available, there may be times when we cannot confirm an appointment at the desired time.

Revisiting Patients

Revisiting patients with appointments

Please proceed to the Revisit Counter on the first floor to obtain a check-in slip for your appointment, then proceed directly to the designated department and wait until the number on your check-in slip is displayed on the electronic board. When your number appears, enter the department waiting room.

  • If your appointment slip is for any of the following departments, please proceed directly to the department without checking in at the Revisit Counter on the first floor: Rehabilitation, Pediatrics, Oral Surgery, Psychiatry, Dialysis, Kidney Clinic, Well Baby Clinic, Palliative Care Clinic.
  • If you are scheduled for testing before your consultation with a doctor, please go directly to the laboratory after checking in at the Revisit Counter on the first floor. After completing the test, please go to the department and explain you have taken the required test.
  • If your appointment involves more than two departments, be sure to let the next department know you have completed your visit to the preceding department.

Revisiting patients without appointments

Present a registration slip to the counter of the department you will be visiting. You will be given a slip bearing the name of your physician and an appointment number. When your appointment number is displayed on the electronic board, please enter the department waiting room.

  • For Pediatrics, check in for revisits at the reception counter in the Pediatrics Medical Center.
  • If your consultation finishes before 11:00 A.M. and you wish to visit another department, please proceed to the department and ask the receptionist if a doctor is available to see you.

Clinic ID Card

If you did not bring a clinic ID card, a new one will be issued at the Document Changes Counter .

Children or Infants

Parents should bring their children or infants directly to the Pediatrics Medical Center for appointments with Pediatrics, Pediatrics Surgery, or the Well Baby Clinic.

Japanese Health Insurance Card Validation

Insurance cards are revalidated once every month at the Revisit Counter  on the first floor. If your insurance card has not been revalidated for more 3 months, the charges for your visit may not be covered under the insurance plan. If there are any changes in your insurance, name or address, please inform us at the Document Changes Counter  on the first floor.

Medical Record Documentation

If you require copies of medical documents, such as the physician's statement, please inquire at the Documentations Counter , on the first floor. Please note that different charges apply to each document and processing times can vary, some types taking weeks. For all other documentation requests, please inquire at the Documentation Department.
